Description

A journal to help you to feel empowered and take control of your treatment journey so that you can focus on what's important . . . you!

*Please note that this item is lovingly Made To Order and will take up to 5 working days to arrive at a UK delivery address (international deliveries will take longer).*

Going through any treatment can be difficult. It disrupts daily life, can be stressful and may be painful. As well as care from medical specialists, family and friends, it’s important that you practice self care.

This journal has been developed by NHS staff and tested by their patients. With instructions to help you get the best out of the journal, you’ll be guided on what information to complete and when, to ensure you’re prepared for your hospital visits and so that your medical team understand your day-to-day experiences and challenges.

There’s space for you to record the medication you need to take and when, the contact numbers and details you may need as well as a daily record sheet where you can make a note of any questions you may wish to ask at your next appointment.

While we’d rather that no one needed to purchase this product, our aim is to help people as part of their self-care programme.

*Please note that this item is printed and hand-finished especially for you and therefore can’t be returned.*

Made with paper & love, from you to me.

Why we love it

We love the fact that this product has been developed by a nurse who has helped many, many people with their long-term treatment. She also asked a number of people to use it and give feedback during its development to ensure it’s the best it can be.
